Our study aimed to provide reference values for five vital signs—systolic and diastolic blood pressure, respiratory and heart rate, and oxygen saturation—in patients over 65 years old presenting to EDs.

We hypothesized that these reference values differ significantly from those established for the general population. Using data from the EDEN (Emergency Department and Elder Needs) cohort, which includes 23,278 ED visits across 52 Spanish hospitals, we identified optimal thresholds associated with death or ICU admission within 72 hours of arrival. Optimal cut-off points (number and values) were derived using the BackAddFor algorithm within a univariate logistic regression framework by maximising the area under the receiver operating characteristic curve (AUROC). These points were validated in a cohort of 37,947 ED patients. Results indicated higher detection rates for adverse outcomes compared to classical thresholds. Our findings suggest that these revised values could enhance the accuracy of ED triage for older adults.
