Chronicity, Primary Care, and Health Promotion

The main objective is to bring research results into daily healthcare settings to improve primary health care, address health chronicity and patient outcomes. The program follows key principles of primary care: providing continuous and comprehensive care, ensuring equity, being easily accessible, and centered on the person and their family. A gender perspective and sustainable implementation strategies are incorporated into all its objectives, ensuring that healthcare solutions are inclusive, fair, and tailored to meet the specific needs of all individuals.

To characterise, evaluate and analyse the impact of health determinants, health promotion and primary prevention strategies at the primary care and community level.

To enhance early detection, optimize diagnostics and therapeutics in non-communicable diseases relevants in primary healthcare, and promote patient safety.

To develop and evaluate tools for early detection of non-transmissible diseases in primary care, and screening strategies and interventions for a secondary prevention of such diseases.

To characterize, compare interventions, and evaluate the care of patients with chronic diseases, multimorbidity, and frailty in primary care, encompassing both the continuity of care processes, longitudinal aspects, and effective access as well as low-value care

To promote innovative and responsible research aimed at translation and implementation of evidence-based clinical practices, focused on personalized care incorporating the gender perspective and patient involvement, to enhance health outcomes and care coordination.

Design, application and evaluation of implementation strategies aimed at enhancing the reach, adoption, implementation and maintenance of best practices and evidence-based health promotion and prevention interventions to enable generalizable and sustainable health outcomes in Primary and Community Health Care setting.

To develop and evaluate patient-centered care strategies and interventions, networks of RWD and digital health solutions, as well as quantitative and qualitative methodological improvements for health services research.

To generate population-based RWE and RWD networking structures within the Spanish National Health System to support clinical decision-making in chronic, prevalent disease in primary care, as well as development and appraisal of e-health solutions to improve care delivery.
